Keyhole surgery can control acid reflux and repair a hiatus hernia when tablets no longer help. Most people go home the same day and notice relief from heartburn and regurgitation soon after.

Overview

Gastro-oesophageal reflux disease happens when stomach acid escapes into the oesophagus. This can cause heartburn, a sour taste, coughing, or voice changes. Many people improve with lifestyle measures and medication. If symptoms continue despite these steps, surgery can restore the anti-reflux valve and repair any hiatus hernia. The operation is performed through small cuts in the abdomen under a general anaesthetic.

Who surgery may suit

Surgery is considered when reflux persists despite medication, when tests confirm reflux or a hiatus hernia, and when you are fit for a short inpatient stay. Your surgeon will review your history, any endoscopy and pH/manometry results, and discuss the right approach for you.

Treatment options offered

The main procedure is a laparoscopic fundoplication. The hernia is repaired and the top of the stomach is gently wrapped to strengthen the valve. The exact wrap (for example Nissen or Toupet) is chosen based on your swallowing tests. Some patients have a transabdominal anti-reflux operation using similar keyhole techniques.

Benefits, risks and recovery

Surgery offers high rates of symptom control and often reduces or removes the need for long-term medication. It is normal to feel bloated or slightly tight when swallowing for a short time while swelling settles. Less common risks include bleeding, infection, troublesome difficulty swallowing and, rarely, recurrence of symptoms. Most people follow a soft diet for about two weeks, return to desk work in one to two weeks, and avoid heavy lifting for four to six weeks. You can drive again when you are comfortable and no longer need strong pain relief.

What to expect on the day

You will have a pre-assessment and time to ask questions. The operation usually takes around an hour. After the procedure you will be encouraged to mobilise early. Most patients go home the same day or after one night.

Aftercare and follow-up

You will receive written advice on eating and activity, and a follow-up appointment to check recovery. If symptoms persist or return, further tests can be arranged and options discussed.
